New Patient - What to Expect

For your first visit, you should arrive 15 minutes prior to your scheduled appointment time. You may be required to complete additional paperwork. If you are unable to complete this paperwork on your own, it is recommended that you bring a family member or friend to assist you.

On your first visit, you are required to bring all of the following:

  • Photo ID (current)
  • Insurance Card/s
  • All medications, including the bottles, which you are currently taking
  • Testing results – Blood tests, MRIs, CTs, Xrays
  • Medical Records from previous doctors and/or your primary care physician

You will be asked to verify the accuracy of your personal information at each subsequent office visit. Please bring your insurance card and photo ID to each and every visit.

Copays are expected at the time services are rendered. We accept the following methods of payment: Major credit cards (American Express is not accepted at this time), cash and personal checks.

While every attempt is made to ensure accurate appointment times for all patients, please understand that there are instances where the doctor may need to spend unexpected additional time with another patient, thereby causing a delay in your appointment time.
